Support Coordination after things change suddenly — Rooty Hill
Coordination of Supports specifically includes resolving points of crisis — here's what that looks like in practice. We also cover nearby Mount Druitt, Minchinbury, Plumpton.
The three funding tiers
This category can be added at your initial planning meeting or at a later plan review if your circumstances change — it isn't a one-time decision locked in forever.
Support Connection
The lightest-touch tier — help understanding your plan and connecting with mainstream, community and funded supports.
Coordination of Supports
More hands-on: building your ability to coordinate supports yourself, resolving points of crisis, and strengthening your connections in the community.
Specialist Support Coordination
For higher-complexity situations, provided by a specialist practitioner — usually where there are complex barriers or risk factors to manage.
Practical, human support.
